• The monitoring and record keeping fee is $20. Cash, Personal Checks, Money Orders, Visa, Master Card, and American Express are all acceptable forms of payment.
• You must register and pay with the Safety Council SWLA before you will receive your church cards.
• You are to attend church for the full length of time as ordered by the court. Although you are welcome and encouraged to attend as often as you would like, you will only be credited for one signature a week. (For example if you attend church on Sunday and Wednesday during the same week, it will only count as one week of attendance.)
• Procedures to get cards signed:
  1. Print your name and docket number on the front of the card. If we cannot read the name and/or docket we will not be able to credit the correct person.
  2. Present the card to the minister or other church official before the service begins.
  3. Ask them to date, sign, and put a phone number on the card to allow us to verify the signature (you do not date your own card ahead of time).
  4. Have the clergy member return it to you after the service.
  5. Mail or bring in your cards each month to the Safety Council SWLA
• Once your initial set of cards is received, any additional cards will be $5.00. The Safety Council WILL NOT ACCEPT FORGED SIGNATURES OR ALTERED CARDS. Signatures must be on official church cards from the Safety Council only, all others will not be accepted.
