• The fee is $50. Cash, Personal Checks, Money Orders, Visa, Master Card, and American Express are all acceptable forms of payments.
• You must register with the Safety Council SWLA and pay for the assessment before the appointment can be scheduled.
• Appointments are scheduled at time of payment.
• If you need to reschedule your appointment you must call 24 hours prior to your appointment or a $10 reschedule fee will be charged.
• The assessment is an interview that is completed to help identify people with substance use disorders.
• We currently use the Substance Abuse Subtle Screening Inventory-3 (SASSI-3) as our screening instrument.
