Contractor Application Guide
The Contractor Safety Recognition Program is a means of recognizing contractors who have exemplary safety programs and initiatives and have demonstrated outstanding safety performance. Continuous improvement in safety programs, initiatives and performance is an important element of every successful company. The elements and administration of this revised program is intended to recognize those companies that demonstrate continuous improvement.
The Southwest Louisiana Contractor Safety Recognition Program was upgraded to include input from area Contractors, the Southwest Louisiana Construction Users Council (SLCUC), the Lake Area Industrial Alliance (LAIA) and the Safety Council of Southwest Louisiana who all serve as co-sponsors of the program. Our awards ceremony will be held later this year to recognize the 2009 Contractor Safety Performance award winners. In addition the awards will be recognized in the newspaper and other news media. Those contractors who achieve award recognition will have the name of their company and the year of the award permanently displayed on a plaque to be displayed in the main Board Room at the Safety Council where the Lake Area Industry Alliance and Southwest Louisiana Construction Users Council meet on a regular basis. The contractor will receive a plaque that can be displayed in their office.
The Awards Ceremony is currently under development by a special committee of local Contractors. The objective is to significantly upgrade the Ceremony with a best practice sharing and presentation for all contractors as well as our local Industrial Participants at the Ceremony. Seating will be by reservation and we anticipate a large turnout of local management personnel and area contractors.
IMPORTANT NOTICE
Please review changes to our program and applicant requirements below:
- Notice applications must be received no later than March 31st to be accepted.
- Contractors self nominate for consideration and will compete, not with peer companies, but with high standards established by the Contactor Safety Recognition Committee.
- A standard application form (go to the Safety Council web page for an application), safety audits and owner letters of recommendation will provide the committee with the necessary information to perform their evaluation task
- Applicants must have worked a minimum of 5000 hours of work at an LAIA member company during the year the award is earned.
- OSHA and EMR data must be drawn from work performed in the local area, not corporate data. (See #4 below under Program Details)
- Contractor work categories:
General Contractors:

Category I – 5000 to 100,000 man-hours annually

Category II – 100,001 to 500,000 man-hours annually

Category III – Greater than 500,000 man-hours annually
Specialty Contractors:
- Category I – 5000 to 100,000 man-hours annually
- Category II – 100,001 to 500,000 man-hours annually
- Category III – Greater than 500,000 man-hours annually
Program Details:
1. Contractor self nominates by completing the standard application form.
2. The application must include a list of LAIA sites where work was performed during the year of the award recognition. (LAIA web site www.laia.com )
3. The contractor must identify a specific local contact person regarding the Safety Award. E-mail address and phone number are required so the committee can follow up with questions during the evaluation process.
4. The program is based on the most recent calendar year with a 36-month look back at EMR, Total OSHA Incident Rate, OSHA Citations and Fatalities. All data on the application must be based on statistics for the company work performed in Southwest Louisiana or the division of the company that has performed the work in Southwest Louisiana.
5. Contractor will provide a minimum of two letters of recommendation from owners for whom they have performed work during the calendar year under evaluation. One of these letters must be from an SLCUC member company. Letters submitted must be the signed original. SLCUC member companies are listed on the membership page of the LAIA web site at www.laia.com
6. To be considered for recognition the contractor must have a current PQF and a current PSM Audit.
7. If the date of the most recent PSM Audit is greater than 12 months the contractor must immediately schedule a review audit which is paid for by the SLCUC.

It is critical that audits be scheduled and completed before the program evaluation team meets to review all applications on May 15th.
8. Completed application should be submitted in an 8 ½ x 11 inch three ring binder. The following must be included in the binder in the following order. Please separate each item in binder with a labeled index tag:
- Original application signed and dated by an officer of the company.
- Copy of OSHA Logs for the three years reflected in the application.
- List of owner companies worked for in the most recent calendar year of the application. Include company name, contact person and telephone number.
- Note that the appearance, organization and completeness of the application is a reflection of the level of importance that the applicant gives to the safety recognition program.
Special value will be given to those companies who provide detailed supplementary documentation of any special or unusual best practice programs the contractor has that encourages and supports employee involvement in their Safety and Health programs. This includes, but is not limited, to areas such as; behavior based safety programs, safety involvement incentive initiatives, risk identification programs, job site safety meetings to review hazards, comprehensive Drug & Alcohol abuse prevention programs and safety training programs beyond the minimal required as well as Supervisor training and development.
Winners may be required to present their best practice to an audience of peers and owners.
11. All applicants will receive feedback from the Safety Awards Recognition Committee on the assessment of their application and ways to improve performance.
12. Applications must be received not later than 3:00 PM, March 31st. They may be mailed to the Safety Council of Southwest Louisiana at 1201 Ryan Street, Lake Charles, LA 70601 or hand delivered to the attention of R.J. McCorquodale at the same address.
